This is a simple answer - you should, if you are involved in any part of the assessment process. t is your responsibility and ultimately all of our responsibility to ensure that SAP is carried out successfully for the benefit of service users and carers.

A person who carries out the Contact and Overview Assessments should be experienced, suitably trained but not necessarily qualified.

Make a note here of all the agencies or departments (ie reablement, care direct, wards) who could be involved in SAP in your area.
